Cashless Insurance Cashless in-patient treatment is subject to the approval
from the TPA solely as per their instructions.
The patient or his / her relatives are advised to inform the office regarding Mediclaim
and TPA prior to admission and to get pre-authorisation form. The Pre-authorisation
form should be filled carefully and duly signed by the insured and treating doctor.
In case patient fails to inform the TPA/Insurance Company within the stipulated
period or there is a denial from the TPA/Insurance Company, the patient will be
treated as normal cash paying patient.
The above mentioned list is subject to change at any point of time. You are requested
to confirm the same from our TPA desk.
Documents required for cashless hospitalization:
- TPA Card
- Policy Copies
- Id Proof of Patient
- If corporate policy: Corporate id card
- Positive Investigation reports
- Doctor's Consultation papers
- MLC/FIR copy,previous discharge card:if applicable for particular case
